I cannot make out the name of the cemetery, shown in the lower right-hand corner of the document.

I looked for lists of cemeteries in Fleming County, Kentucky, but couldn't find anything that looked like that which started with J. If I sit back and look at it the closest town name I can think of is Danville, but Danville's in Boyle county, which is a ways off from Fleming -- and that also doesn't explain why the front letter looks to be a J.

I also got no results when I googled for "Hillston, Kentucky", as the undertaker's address. There may be a Janville, Ky but Google wants to give me Danville instead.

Jackson Graveyard in Fleming County does have a Sarah Jones buried there, apparently. But the dates are wrong. Perhaps they're family, though; there are several Joneses.
